Adding a "none of the above" option to a list of checkboxes
tl;dr Make the form behave as if the user is filling a paper sheet while someone stands beside them to nag them when they make mistakes.

I'd flatten out multi-level stuff in a single linear sequence of steps, eventually skipping forward/backwards more than one as needed, I'm thinking something similar to Microsoft SQL Server setup.
