@KarolyS: I just did some more reading and I doubt "ethnicity" is what you want for medical purposes. People who share a culture and language but are of different races can have the same ethnic. I think you are really looking more for ancestors origin regions or continents. Not sure how to say it.

@David: Unless it is really important to them I would bet they just stab randomly at whatever is close enough. Perhaps because I am Caucasian I just don't care about it? Should I worry that I can't enter English/Russian/German with the appropriate percentage mixes?

As a guy who has done data entry in the past I want forms to be consistent and fast. Consistent! I want to go through a list of items as fast as possible. I don't want to read anything. I don't want questions. I don't want mysterious system delays. I want entry, Tab, entry, Tab, entry, Tab, DONE! Pow, next form.

I like your tips, except for avoiding dashes. If you can get the domain name with dashes and without dashes. Dash separating the words makes it much more readable.

I believe another example of your point is a Land Rover. It could be curvy like a Porsche but that would not look right for its purpose.
