why does Mailing Address have 4 Line fields? 2 seems the standard. Also, I'd probably go First, MI, Last for name. Also, what about Prefix (I've seen those more than Suffix)?

comment Why do progress bars animate backwards?
No need to summarize a study. Just think for a second... The main anchor point is the front of the progress. A reverse animation makes your brain create mini anchor points from the animation. Because mini anchor points move left and the progress moves right, it seems like the distance is getting larger. Hence, the bar appears to be moving faster.

@Marcos: the bathroom probably doesn't have proper ventilation. (Usually in extreme cold temps, their is little moisture, so I doubt the cold waves are the prob.)
