@MonicaCellio: you should not press the clutch for the whole red-phases because this will increase the wear of the clutch. It should be enough to hit the clutch and insert the first gear when the crossing pedastrian's traffic lights turn red.

Your differentiation between Close and Cancel is IMHO incorrect: Close is used to close something (e.g. a dialog), even if something has been changed. Cancel is used to ignore changes done in the opened window.

Regarding avoid changing the menu bar at all: our application can show multiple top-level window for significantly different objects. I'm not sure it would be a good approach to merge everything into one stable menu bar (see also ux.stackexchange.com/questions/10251/…).

Imagine you have a parent-modal dialog open to input further details which are required to perform a command in the first top-level window. Why it should not be possible to open a new (second) top-level window to find this information easily or perform a different command?

Regarding your auto-save suggestion: The items are indeed text files which usually are edited outside of our application. I don't know of any text editor which automatically saves files to disk. Also, we have one viewer now, I don't want to turn our application window into a whole text editor which can have dozens of edited files open. Hence we need to limit to this one visible viewer/editor.

Regarding your ways 1. and 2. I've updated my question. BTW, for dialog boxes with OK and Cancel buttons I would NOT expect that the item's content is saved immediately, but first when I press the OK button. Beside that, we don't use (modal) dialogs, but windows and don't want to change that.
