I'd recommend you read this quora answer by a guy from Netflix - quora.com/Rating-Scales/…. Ratings are only a feature if you use them properly. Incidentally, users only give you valuable data if you use them properly, too.

Amen on not customizing links. Users can decide client-side how they want their browser to handle PDFs. Don't make (or worse, unmake) the decision for me.

I really think this question should be posed on security SE. There are lots of people who contend that security questions amount to a backdoor for accounts. If you're designing something from scratch you're probably better off implementing two-factor authentication.

Why not simply hide the N/A flags? Rather than displaying the full list and figuring out how to indicate what matters, how about only making the displayed list include what matters and have functionality whereby people build the filter from available list items?

I personally do it more to express my anger at the machine. Sort of a "take this" action. I know this because my tapping on the spacebar isn't designed to be functional, it's designed to beat the crap out of the keyboard.
