Just edited the mockup... let's suppose all options support paste from clipboard.

comment Best UI to allow expert users to enter an ip address? (IPv4)
Chris, I am leaning toward option 3 since our target user is an it administrator. They type in IP addresses all the time. And if we can give instant, visual, validation feedback, I feel like it's not a bad option.

comment Which UI/UX sketching software is recommended?
Those are really pretty sketches. There's no way I could accomplish something so legible without software unless I spent a LOT of time. Software is way quicker for me. I will at times resort to a whiteboard in order to communicate something to another person in less than 10 seconds.

He's asking about software. This is a silly answer.

comment Should a button become lighter or darker on hover?
I don't have any data to refer to, but my inclination is that what really matters is that 1) you are consistent and 2) the button (and/or cursor) changes enough to be noticeable.

comment How did you create design guidelines for your organization?
This comment is completely tangential, but, you don't need to put radio buttons inside of a label tag in order to make the text trigger a state change. If you use the for attribute on a label and specify the radio button id attribute value, then it will do the same thing.

comment Credit Card input format
If you go the route of 4 separate boxes with auto-tabbing, then make SURE that people can fill out a box, go to the next box and return to a previously filled box without being auto-tabbed to the next box. This is extremely frustrating when I need to make a correction and the auto-tab logic is "too smart" for my own good.

Why does it need to autotab?

comment Auto focus on the first form field; done right?
Few things are more frustrating than when I begin filling out a form on a slow-loading page and then my focus is taken away from me when the page is done loading. So if you're going with a javascript solution, add some logic to see whether the user has already started typing. If so, do nothing.

comment In which cases does it make sense to use an explanatory video on a homepage?
I don't think technical "hurdles" should be a reason not to use videos, because there are really easy ways to host them. Your other points are valid.

comment Should double click be avoided in web applications?
+1 I recently created an internal web app which uses this strategy. I would be very hesitant to make a public-facing website where double-clicks were required.

Love the suggestion on building the CMS so that they can choose which payment methods to use.

That's a non-answer.

comment Is it a bad idea to have input fields after password on a login form?
I would add that IF you include a "Remember me" checkbox in between the password and submit button make sure that the tab sequence corresponds to the visual layout. Sometimes I experience login forms that skip the "remember me" field in the tab sequence in this scenario thinking that it will help when it is more harmful.

comment Is it a bad idea to have input fields after password on a login form?
@Marjan, that's the way it SHOULD work, however there are lots of websites out there which have been poorly made using asp.net webforms that don't handle form submissions via the Enter key properly. Because of this, I almost always use the pattern @DanWilson describes.

I just finished this book. It was VERY worth my time.

comment Should I include the month name in my select list for credit card expiration month?
I see your point here. But @DanM raises a really good point that some users would take that to mean "August 8th". Since most cards tend to format their months with a leading zero, I would want to keep it. I could possibly create a JavaScript solution to achieve what you propose which would allow most people to type the month.

comment Should I include the month name in my select list for credit card expiration month?
I have accepted your answer because I think it would have the most usability to most people internationally. We have a product that sells in several countries.

I actually prefer using text boxes too. I seem to recall a particular brand of credit card which uses actual month names on it, but I can't verify that.
