comment User friendly and clear way to present possible answers to a question
When I see scenarios like your second image for 'responsive enabling', I always get confused because I'm not sure if it's saying "Yes and also A,B,C" instead of just "Yes". So I always select "No", deselect A,B,C, and reselect "Yes", which is highly frustrating.

No they're all unique and unrelated, unlike the example I posted above. There's no parent-child hierarchy.

comment Should I ask users to rate my app with pop-up window?
Er, what? Who said anything about free apps? If you show an intrusive popup saying "RATE MY APP!", I find it irritating. It's as if I were walking down the street and someone stepped in front of me and thrust a sign in my face. I have to stop and deal with it, and since I'm now irritated it probably won't be favorably. However if it's done seamlessly or unintrusively, I won't mind -- Huffington Post or another online news site recently started showing a one-question survey as a "answer this and then keep reading" sort of thing. It had a very obvious 'decline' option. I actually answer those.

comment Should I ask users to rate my app with pop-up window?
I always give them the lowest possible rating if they interrupt my browsing that rudely. Not scientific or indicative of anything, of course, but there will probably be others who do the same.

comment Should “Yes, delete it” be red, or green?
@Jeremy - Out of curiosity, could you tell which one's which if you didn't have text on the button?

comment Should “Yes, delete it” be red, or green?
@Lohoris, ChrisWue -- According to the National Institute of Health (US), it's "about 8%" of men. Wikipedia says 7-10% of men. I misremembered my statistics.

Neither, if you're red-green color blind like 10% of men.

comment Most suitable text for “Saved Queries” for non-tech users
How do you identify the filters in the save option? Do you say something like "save this search for future use"? The option for using the saved preferences should reflect the language you exposed previously. Eg. "save these search options" -> "apply search options (followed by a drop-down/selector)".

comment How could you prevent users from leaving their original copy in the photocopier?
@Urbycoz - I've never seen any body part involved in Christmas party shenanigans get left on the photocopier. Unless our Christmas party shenanigans are of a different caliber than the norm...

If you include a link to your drawing, a user with more experience will usually be willing to come by and inline it for you. Welcome to UX.SE, by the way.

comment On Mobile Web, what's the best way to present Date of Birth form fields?
As an American user, when I see a prompt for DD/MM/YYYY I have a mental hiccup and have to stop and think. It's an extra (minor) cognitive load.

This would also need to support both mm/dd/yyyy and dd/mm/yyyy based on user preference. yyyy-mm-dd may be a better, neutral format.

comment Is there a better character than the asterisk to indicate a modified document?
Wow. I just learned why that strange dot character shows up next to the file name when I use Sublime Text 2.

comment What steps should I follow designing a web site?
In addition to all the answers below, an important first step is to consider who your target audience is. What is the demographic, how literate are they, how much time do they have to spend, how likely are they to spend money/scroll/read lots of text/do xyz, etc.
