comment Visual language for fixed and fluid widths
I agree that you might not want to specify exact how wide the columns should be. That is something that the web designer should decide.

comment Usability research on sticky headers?
Personally, I really dislike the concept. I always find it irrating except on Facebook and Twitter for some reason. Maybe because it's kept to a minimum.

comment Should we include a “Back to top” button for mobile websites?
Yes, that is correct. The question is about accessibility.

comment How do left-handed people hold smartphones?
I'm left handed and I always use my left hand and my left hand thumb to navigate and write. Both in landscape mode and portrait mode. If I need to pinch (for example to zoom) I use my right hand while holding it with my left.

comment Is there a convention for identifying final vs. placeholder text in wireframes?
I think they could be. Sometimes we perform usability tests on wireframes and in those cases text is important. Also, it's a good idea to hint the amount of text we want to use.

comment What reasons are there for “go to bottom/top” links no longer being used much?
Since a Back-to-top link is very easy to implement you could just add an event in Google Analytics and check how many people do click on it. If no-one does then remove it. (provided that the link is fairly visible)

Hmm, I'm not sure what you mean. Could you rephrase your question?

comment Most intuitive settings icon
IMHO - the main problem with the hammer and the wrench is that when you need to use the icon in a smaller size (such as 8*8 or 16*16 pixels) they become pretty unrecognizeable. Therefore it's better to only use the cog wheel or the wrench.
