Along the lines of the $300,000,000.00 button mentioned in Luke Wroblewki's book Web Form Design: Filling in the Blanks. Also here, http://www.uie.com/articles/three_hund_million_button/

Also interested in the opposite, i.e. how much is gained as a result of good design.

It doesn't look like you have a particular question here that you need an answer too, only a request for some examples. You won't really get useful answers without a specific question, and a Q&A site isn't the best place to request examples of 'X'.
